Know Your Numbers

Know the numbers that can have the most dramatic effect on your health:  cholesterol, blood pressure, glucose, and body mass index (BMI).  Enter this information into your online Personal Health Assessment and get your wellness scorecard and personal report.  This process will help you become more aware of the aspects of health and wellness that may need some attention, as well as the areas where you have found a nice balance.

STEP 1:  KNOW YOUR NUMBERS HEALTH SCREENING EVENTS

What is it?
All health screenings are provided by a registered nurse with Maxim Healthcare Services. Each appointment lasts approximately 15 minutes and includes a consultation to explain your results. Comprehensive screenings include cholesterol (total & HDL); glucose (blood sugar); blood pressure, body mass index (ratio of height to weight), and body composition (% body fat) and are offered at a variety of locations throughout campus, several times a year.  

Screening methods
Cholesterol and glucose are measured with a single finger prick. All other screenings are non-invasive. Specialy screenings, such as prostate cancer, skin cancer, or osteroporosis are offered at least twice per year. Prostate cancer screnings (PSA) are also measured with a single finger prick.

Appointments
Appointments are highly recommended and may be booked on-line using the link provided. All health information is completely confidential.

Your personal information is protected
All information is collected by a third party vendor (Maxim Healthcare Services). Maxim maintains strict confidentiality of all participants information in accordance with laws regarding the privacy of personal health information, including HIPAA.  AU does not have access to any personal health information.  The only data AU acquires is number of individuals who choose to participate in the day's event.

Access
Health screenings are offered on campus for your convenience. Alternatively, you can schedule an appointment with your primary care physician for an annual physical.

Kaiser Permanente members who have already seen his/her physician for an annual physical may have access to their information online through My Health Manager at kp.org.

Keep track of your numbers from year to year
Everyone who participates in a Know Your Numbers Screening event will receive a Know Your Numbers journal.  The journal is provided so that you may track your results annually.  It's helpful to have this information handy when you take the online Personal Health Assessment.  

Alternatively, you may take the online Personal Health Assessment FIRST, and bring your wellness scorecard and personal report with you so you may discuss the results with a nurse.  Be sure to update your Personal Health Assessment with your screening values for the most accurate report. 


STEP 2: PERSONAL HEALTH ASSESSMENT

What is it?
The online Personal Health Assessment is a questionnaire used to assess overall health and well-being.  The assessment takes about 20 minutes to complete and asks questions on topics such as physical activity, nutrition, vehicle safety, stress, sleep, alcohol use, and men's/women's health.  Upon completion, you'll receive a wellness scorecard and personal report that includes information and action steps for optimizing your health.  The purpose of this tool is to better understand your health status and identifiy areas for improvement to keep yourself well.

What you'll need
For the most accurate report, we recommend that you have your screening values (cholesterol, glucose, blood pressure, and body mass index) handy from a Know Your Numbers Screening Event or from your last annual physical.  If you don't have this information, no need to worry.  You can either complete the assessment without them, or even enter the screening values at a later date.  You'll always have access to the tool and your personal report.  

Instructions for logging in
First time users only:
Be sure to enter Login ID "health" and Password "wellness" on the welcome screen. Next, you will be asked to enter some basic information before creating a unique Login ID and Password. This does not need to be the same as your Novell username and password used to log on to the AU server. Use your newly created Login ID and Password whenever you return.

Returning users:  Enter your unique Login ID and Password or click on Forgot Password? for assistance.

To receive your gift:

1. Complete the online personal health assessment and print the LAST page of your personal report titled "personal goal diary". If you exit the assessment before viewing or printing your report, you may log back on using your unique Login ID and Password.

2. Write your name, campus mailing address (department + campus zip), and choice of gift card on the BLANK page. DO NOT complete the personal goal diary before sending. This is only a means of verifying completion of the personal health assessment.  Alternatively, you may send a screen shot of the final page along with your gift card selection and campus mailing address via email.

Eligibility
At this time, "thank you" gifts are reserved for AU faculty and staff only and are awarded once per calendar year.  We do, however, encourage you to update your personal assessment at any time.

PROTECTING YOUR PERSONAL HEALTH INFORMATION

All information is collected by a third party vendor (Maxim, screenings and American Institute for Preventive Medicine (AIPM)/Trale, Personal Health Assessment). Both parties maintain strict confidentiality of all participants information in accordance with laws regarding the privacy of personal health information, including HIPAA. With regards to the online health assessment, all personal information is enrypted and stored using secured socket layer technology. American University does not receive any personal health information that will identify you as an individual. Your data is stripped of all identifying information and combined with others in a large statistical database for aggregate reporting. This data will allow AU to better understand overall population health for the purpose of developing and implementing preventive health programs and services.
